Play On Ottawa 2017 Ball Hockey Tournament sign-up!

Please use this form to register your player for the Play On Ottawa Ball Hockey tournament on Saturday, June 3 from 12-1pm at the Rockliffe Flying Club!

•This is a free event!

•One form per player, please.

•We have just 30 spots available, so don’t delay – we will follow a first come, first served policy on sign-ups.
